Since Asia is so large in its area and so heterogeneous in its peoples' ethnic backgrounds, the problems of haemoglobinopathies are uneven. Information is not evenly available from all Asian countries for obvious reasons. While haemoglobin (Hb) S is the hallmark of Africa, in Asia Hb E is. Its frequencies exceed 50 per cent in certain areas. Double heterozygosity of p-thalassaemia and Hb E alleles causes more problems than homozygous p-thalassaemia in a vast area of Asia. Hb Constant Spring is far more common in Asia than in any other continents. Hb Bart's hydrops fetalis, which is totally lethal, is almost limited to the S.E. Asians and the Southern Chinese. Hb H disease is far more common in the East Asians than anywhere else. Because of coexistence of several thalassaemia and related genes in the same area, S.B. Asia witnesses the most complex thalassaemia syndromes unparalelled by other parts of the world. Poverty, ignorance, infectious diseases and poor medical care prevailing in the 'thalassaemia belt' of Asia aggravate the sufferings of the thalassaemics. Successive wars in Asia have been dislodging a great number of the people with high frequencies of Hb E and thalassaemias around the world, the most rapid gene migration in history.
