บทคัดย่อ
The proportions of hemoglobin types may be affected by genetic and environmental factors. The levels of hemoglobins A 2 and E were determined in 172 patients with iron-deficiency anemia. Of the 139 hemoglobin AA patients, whose average hematocrit was 18 per cent, the values of hemoglobin A 2 were 1.76 ± 0.4 S.D. as determined by the cellulose-acetate electrophoresis in 126 patients and were 1.73 ± 0.52 S.D. in 129 patients when estimated by DEAE-Sephadex column chromatography. The differences from the normal hemoglobin A 2 value of around 2.5 per cent were highly significant. The correlation coefficients of the initial levels of hemoglobin A 2 and hematocrit and of hemoglobin A 2 and serum iron were + 0.352 (P < 0.0006) and + 0.15 (P = 0.07), respectively. In 25 iron-deficient hemoglobin AE heterozygote patients whose mean hematocrit was 17.5 per cent, the proportion of hemoglobin E was 13 to 25, averaging 19 per cent, in contrast to the 25 to 30 per cent in AE persons who are not iron deficient. The correlation coefficients of the initial levels of hemoglobin E and hematocrit and of hemoglobin E and serum iron were + 0.182 (P = 0.36) and + 0.195 (P = 0.4), respectively. Eight persons, who were both heterozygous for β-thalassemia and iron deficient, had an average hematocrit of 24 per cent and a decreased amount of hemoglobin A 2. No significant alteration of the alkali-resistant hemoglobin was detected in all the patients. Follow-up examinations during oral ferrous sulfate therapy revealed a definite restoration of hemoglobins A 2 and E in corresponding groups toward the ratios observed in noniron deficiency.
